Avery Street Christian Reformed Church wants you to be informed of our intent and possible use of photographs and videos that may contain images of your child or you for promotional materials like pamphlets, church bulletins, websites, newsletters, etc.
If you do not want photos/video/ sound of you or children to be used, please print out the form below, complete it, and return the completed form to the church office or place in the mailbox for Avery Street Church in the church building. The form also contains a detailed description of our policy.
